Permanent housing to end long-term homelessness

Our permanent housing program serves individuals and families that have experienced long-term homelessness and also have a disability. 

The program is comprised of an ongoing rental subsidy alongside intensive supportive services focused on housing and financial stability, health and wellness, and children and family wellness.

Connections’ Case Management Team works one-on-one with each family or individual to assess their situation, meeting each housing challenge with solutions that set them up for success.
