Homelessness Prevention

Your First Step

Call the Suburban Cook Call Center to receive a referral for assistance at 877.426.6515. Alternatively, you can visit one of their walk-in centers. You can find a location through their website at: https://myentrypoint.org/.

The Process

While every situation is different, Entry Point will provide you with an assessment that will determine the services that you and your family is eligible to receive, which may include what we can provide at Connections. While this may feel like an additional layer in the process, this assessment will ensure you are with Connections for the right services so we can address your needs as quickly as possible.

If you’re in a situation where you are unable to receive a referral or have questions about our prevention assistance program, call us directly: 847.475.7070 x134 for the prevention mailbox, or send an email to prevention@connect2home.org.
